How Do I Join?

To join SecureCare (HMO), SecureCare SNP (HMO SNP), SecureCare Capitol Plan (HMO), SecureChoice (PPO) or SecureChoice Capitol Plan (PPO) please read the following information:

STEP 1 – Obtain an information kit

Request more information on Medicare products by calling 1.877.847.7915. For hearing impaired service (TTY/TDD) call 711. You can also complete and submit the Request More Information form.

STEP 2 – Review the material

When you receive the packet in the mail, review the materials in the packet. You are welcome to call a representative at the numbers listed above should you have any questions regarding any of the information.

STEP 3 – Complete the application

You can enroll by mail or online for SecureCare (HMO), SecureCare SNP (HMO SNP), SecureCare Capitol Plan (HMO), SecureChoice (PPO) or SecureChoice Capitol Plan (PPO), and Medicare Supplement Plans. Complete the enrollment form. A separate form must be completed for each person. If mailing, please sign, date and return the enrollment form in the postage-paid envelope. Also, please include a photocopy of your Medicare card, if possible, and a check for your first month’s premium, if applicable. Medicare beneficiaries may also enroll in SecureCare (HMO), SecureCare SNP (HMO SNP), SecureCare Capitol Plan (HMO), SecureChoice (PPO) or SecureChoice Capitol Plan (PPO) through the CMS Medicare Online Enrollment Center located at www.medicare.gov .

STEP 4 – Process your application

We will review your application and verify the information with Medicare. You will receive a letter acknowledging that we received your application and notify you of your proposed effective date. After we receive approval from Medicare, we will send you a membership packet, identification card and formal notification of our enrollment date. If you have supplemental insurance, do not cancel this coverage until you receive notification of your effective date of enrollment.
